Job Description
- Receive, verify, and dispense prescriptions accurately and within approved standards and procedures.
- Verify drug names, strengths, quantities, batch numbers, and expiry dates before dispensing.
- Conduct regular stock-taking and physical inventory counts of medicines and pharmaceutical supplies, reconcile physical stock against system/bin-card records, and promptly report any variances.
- Maintain accurate and up-to-date stock records, including receipts, issues, transfers, adjustments, expiries, losses, and damages.
- Monitor stock levels and initiate timely requisitions to prevent stock-outs, overstocking, and wastage.
- Identify and report slow-moving, expired, damaged, or near-expiry medicines and ensure appropriate action is taken.
- Compound extemporaneous preparations and prepare or mix intravenous medications in accordance with approved procedures.
- Counsel patients on proper medication use, dosage, adherence, side effects, and the use of relevant devices.
- Deliver discharge medications to patients in the wards where applicable and ensure proper documentation.
- Communicate out-of-stock and non-stock items promptly to prescribers and nurses, recommend appropriate alternatives, or initiate arrangements for timely procurement.
- Ensure proper storage, handling, and dispensing of medicines in compliance with pharmaceutical, environmental health, and safety requirements.
- Monitor and regularly update the medication expiry list and implement appropriate measures to minimize losses due to expiries.
- Participate in periodic and scheduled stock-takes, including monthly, quarterly, and annual inventory counts, as required.
- Investigate and report discrepancies identified during stock-taking and assist in implementing corrective measures.
- Ensure proper segregation and documentation of expired, damaged, recalled, or quarantined medicines.
- Complete documentation of interventions, medication errors and near misses, adverse events, medication queries, patient complaints, and other relevant incidents, and forward them to the appropriate office.
- Maintain confidentiality and comply with hospital policies, procedures, and professional standards.
- Diploma in Pharmaceutical Technology from a recognized institution.
- Valid practicing licence from the Pharmacy and Poisons Board (PPB).
- At least one year of experience working in a busy healthcare facility.
- Computer literate, with experience using pharmacy/HMIS systems being an added advantage.
- Good understanding of pharmaceutical inventory management, stock control, and reconciliation.
- Ability to communicate effectively with doctors, nurses, and other healthcare professionals.
- Ability to work effectively as part of a multidisciplinary team.
- Excellent customer service skills and empathy when dealing with patients of all ages.
- Good interpersonal and communication skills.
- Culturally sensitive and respectful when interacting with patients and colleagues.
- Demonstrated attention to detail, integrity, and accountability in handling medicines and pharmaceutical records.
- Willingness and ability to keep up with pharmaceutical developments and participate in Continuous Medical Education (CME) programmes.
