Medical Social Worker III (CPSB 11-3 Positions)

The Medical Social Worker III will be responsible for providing social work services to patients and their families. The successful candidate will be required to work in a team environment and provide support to patients and their families in a variety of settings. The Medical Social Worker III will be responsible for assessing patients' social and emotional needs, developing and implementing plans to meet those needs, and providing counseling and support to patients and their families. The successful candidate will also be required to maintain accurate records and reports, and to participate in quality improvement initiatives.
